Effective March 24, 2020, the MASS Clinic has become a virtual clinic in order to protect our volunteers, providers and patients. We have the technology in place to screen all patients address any immediate concerns via our telemedicine program to limit their need to seek an in-person visit.
Patient must acknowledge that they have reviewed the Informed Consent to Telemedicine Consultation policy of Muslim American Social Services and agree with all of the terms and conditions contained therein and authorize the administration and staff of Muslim American Social Services to affix their electronic signatures on on State of Florida Volunteer Health Care Provider Program and new patient application forms. Eligibility Criteria : Patient MUST be Duval County resident; Age 18 and above; Patient must NOT have any health Insurance; You must bring a proof of income or a notarized letter of support document in case of no income
